Bidirectional peristalsis, dilated bowel loops containing increased amounts of fluid

Paralytic ileus

Absence of peristalsis, dilated bowel loops

Peritonitis

Thickened peritoneum, dilated bowel without wall swelling, free fluid

Less common

Mesenteric vascular
occlusion

Long segmental thickening of bowel walls, acute luminal narrowing, decreased peristalsis

Dissecting aortic aneurysm

Aortic dilatation, floating anechoic band in aortic lumen, double lumen

Rare

Intra-abdominal bleeding

Essentially anechoic fluid surrounding the organs; capsular discontinuity is seen with an organ rupture
